We have laid out the following steps for first time patients, so that you can start your holistic health journey with us! Find which service you are looking for! We have several different services that we provide here at Whole Life Clinic. Read through our different service options and determine which would be best for you. If you aren’t sure, you can always contact us, and we can help you find which service is right for you!
Book your appointment! Once you have decided which service you would like to pursue, you will need to schedule a time that you can come into our office for your appointment! Find a “Book Appointment” button on the website and register for the scheduling service. From there you can select which service you would like to book, and it will give you available times and dates for you to come in! If you have any issues with scheduling, you can always call or email us, and we can schedule an appointment for you.
Fill out the new patient form! We have a PDF of our new patient form (the Metabolic Assessment Form) listed below that you can download and print off to write in the answers, or you can fill it in on your computer. Fill out the form to the best of your ability and then either email it to us ahead of time, or just print it off and bring it with you to your appointment.

We have an online shop where you can buy your supplement refills, but you need to be registered to be able to access it. Fill out the required information on this “Register For Store” form. Once we receive the information we will register you for the store, and you should get an automatic email that has your username and a temporary password. *Note* The username will be just the beginning part of the provided email, not the whole email! (Ex. If your email is work.email123@gmail.com, your username would just be work.email123). Once you receive this email, you can then go to our website and click the “Order Products” menu item located under “Patient Info” to log in. If you have any issues with this, please feel free to contact us. If you do not wish to register for the online store, we do take supplement orders over the phone.
